This is the "Moon Rock" stained glass window at the US National Cathedral in Washigton DC. It is located on the south side of the building, approximately half way down the aisle from the main entrance. There is a moon rock embedded in the window in the large dark circle above center. The glass also shows the path of the Apollo spacecraft from the Earth (dark circle in the lower right) to the moon (the large dark circle with the moon rock).
